Form 1095-C: Notice of Availability
This notice is intended to provide Aires Medical Plan participants with instructions to request a copy of form 1095-C for tax years 2024 and forward under the new requirements set forth by the Paperwork Reduction Act (update December 2024). Forms will be provided upon request.
Aires prepares and files individual forms 1095-C: Employer-Provided Health Insurance Offer and Coverage annually for each employee and medical plan participant within the tax year. Form 1095-C: Employer-Provided Health Insurance Offer and Coverage is a tax form used to report information about an employee's health coverage and also provides monthly enrollment information for all individuals enrolled in the company health plan including dependents. This form is not required to be filed with a tax return, and employees may keep this form for their records. Information from this form can also be used to help determine a taxpayer's eligibility for credits that may be available as they relate to affordable care.
To request a copy of Form 1095-C or for further information, please contact the Aires PeopleX Team via the information below. Aires will provide a copy of 1095-C forms within 30 days of request via US mail or electronically by request.
